The short answer
Many PPO and other private health plans include benefits for medically necessary drug and alcohol treatment, including medical detox and residential care. Whether your plan will help pay for treatment at iVital Wellness, and how much you would owe, depends on your specific policy. A benefits check for your plan answers that before you decide anything.
Why many plans include addiction treatment
Substance use disorder treatment is one of the essential health benefits that individual and small-group plans under the Affordable Care Act must include. The federal Mental Health Parity and Addiction Equity Act also requires most plans that cover addiction treatment to do so on terms no more restrictive than their medical and surgical benefits.
That does not mean every plan pays for every program. Plans still decide which providers they pay, how much, and for how long, based on your policy and on medical necessity.
Accepting insurance is not the same as being in-network
A treatment center can accept your insurance without being in your plan's network. The difference matters for what you pay:
In-network
The facility has a contract with your plan and agreed rates. Cost sharing is usually lower, and the facility generally cannot bill you above the contracted rate for covered services.
Out-of-network
Many PPO plans still pay part of the cost, based on the plan's allowed amount. Deductibles and coinsurance are usually higher, and you may owe the difference between the bill and what the plan allows. HMO and most EPO plans do not cover out-of-network care except in emergencies.
Network status can differ between plans from the same insurance company, so we confirm it for your specific plan rather than relying on the logo on your card.
Costs that can still apply
- Deductible
- What you pay each plan year before the plan starts sharing costs. Out-of-network care often has its own, higher deductible.
- Coinsurance
- Your percentage of the cost after the deductible, for example 20% or 40%. Out-of-network coinsurance is usually higher.
- Out-of-pocket maximum
- The most you pay for covered services in a plan year. Some plans count in-network and out-of-network costs separately.
- Allowed amount
- What your plan decides a service is worth. Out of network, you may owe the difference between the facility's charge and that amount.
- Non-covered services
- Anything your plan does not consider medically necessary or does not include in your benefits.
Your admissions coordinator should walk you through your expected share of the cost before you commit to treatment. Ask questions until it is clear.
Authorization and length of stay
Most plans require prior authorization for detox and residential treatment. The insurer reviews clinical information to confirm the level of care is medically necessary, often using the ASAM Criteria that addiction specialists use to match people with the right level of care.
Insurers usually approve a limited number of days at a time and then hold concurrent reviews with the treatment team. That is why no one can honestly promise how many days a plan will cover before treatment begins.
How a benefits check works
- Step 1
Call or request a callback
Call us or leave your name and number. Have the insurance card in hand if you can.
- Step 2
Share your plan details by phone
An admissions coordinator collects the information on the card and a few questions about what is going on.
- Step 3
We check your benefits
We contact the insurer about detox and residential benefits: network status, deductible, coinsurance, out-of-pocket maximum and authorization rules.
- Step 4
You decide on next steps
We explain what we found and what you may owe. If treatment here fits, a clinical assessment and authorization come next; if not, we can point you to other options.
A benefits check is not a guarantee of payment. Your insurer makes the final coverage decision based on your policy and medical necessity.
If you don't have a PPO
If you have an HMO, an EPO, Medicare or Medi-Cal, or no insurance, contact admissions to ask which payment options are available for your situation. Do not assume that a particular plan is accepted or that treatment will be covered. You can also reach the free, confidential SAMHSA National Helpline at 1-800-662-4357 or search FindTreatment.gov for other programs.
Where treatment happens
Treatment takes place at our residential facility at 15668 Live Oak Springs Canyon Rd, Santa Clarita, CA 91387, in northern Los Angeles County. People come to us from across the Los Angeles area and beyond. If you live elsewhere, whether your plan covers treatment in California is part of the benefits check. See the facility or the areas we serve.

Common questions
Will my PPO pay for treatment at iVital Wellness?
It depends on your specific plan. Many PPO plans include benefits for medically necessary detox and residential addiction treatment, but network status, deductibles, coinsurance and authorization rules differ from plan to plan. A benefits check for your plan is the only reliable way to know.
How many days will my insurance cover?
No one can promise a number of days in advance. Insurers usually approve a level of care for a limited period and then review progress with the treatment team before approving more. Length of stay is based on clinical need and your plan's rules.
Does insurance pay for the amenities at a private rehab?
Insurers pay for clinically necessary care such as medical detox, nursing, therapy and residential treatment, not for comfort features in themselves. Your admissions coordinator can explain what is billed to insurance and whether anything would be your responsibility.
Does checking my benefits commit me to anything?
No. Checking your benefits does not commit you to treatment. It gives you the information you need to make a decision.
Can I check benefits for a family member?
Yes. Many calls come from spouses, parents and adult children. We will need the details on the insurance card, and the person seeking treatment will need to take part in the clinical assessment before admission.
